When you call in to NorCal, just provide the following information and we can pre-approve you over the phone, we will copy the information at your location for our records. Make sure to also review, sign, make a copy of NorCal's P2P Membership Agreement.
Phone Information Required:
Doctor's Name, Doctor's practicing address and phone number, Doctor's license number, Patients(you) Ca. drivers license number and expiration date, Patient ID number and Expiration Date. We can pre-verify you over the phone and verify documents at your location. We will also copy your information for our records at your location.
When we arrive, simply have the Original Recommendation Letter from the Physician and a copy of your Ca. Drivers license or Passport for verification and a signed copy of NorCal P2P membership agreement. Note: It is much easier and faster for membership approval if information is sent to us via internet or fax.
Don't forget to Fill out and Print NorCal's P2P online Membership Agreement read it, sign it, and have it ready when the patient member arrives with your medication.
